It is recommended that a loan application be submitted for review at either L.E.O. office during regular office hours. This will minimize delays or rejections due to missing information. In the interest of time, faxed applications will be accepted for initial approval, however, the original application MUST BE MAILED to the L.E.O. office as soon as possible
| FIRST: COMPLETE THE LOAN APPLICATION, THEN REVIEW IT USING THE FOLLOWING CHECKLIST: |
| 1. Applicant must be a L.E.O. member with a balance of AT LEAST $50.00 IN A SAVINGS ACCOUNT. An appication fee of $25.00 will be withdrawn before the application is processed, and whether or not the application is approved. |

| 2. Applicant must complete the GRAY BOX in the upper-right hand corner, including social security number. |

| 3. Applicaton MUST COMPLETE all of the following information: -Full Name- Including middle initial, Jr., Sr., or III. -Full Address- Including City, State, and Zip Code. -Bi-Weekley Salary -Birth Date-Hire Date (Non Lubrizol employees must provide proof of income in the form of (2) pay stubs). -Complete list of current debts including mortgages, installment loans, and credit cards. |

| 4. If using this application for L.E.O. to pay off other debts, indicate which debts you will be paying off. |

| 5. "Personal" is not a reasons for borrowing Briefly describe the reason for the loan. |

| 6. New or used auto loans, both dealer and private transactions, must include the sales agreement showing the total cost and brief description of the collateral, signed by the dealer/seller and yourself. Boats and RV's must include the same(used vehicles must include mileage or hours). ALL RV purchases MUST BE TITLED. |

| 7. All vehicales must carry collision insurance throughout the course of the loan. Complete the form regarding your insurance company, agent's name and phone number. VEHICLE LOANS WILL NOT BE PROCESSED WITHOUT THIS INFORMATION. |

| 8. If a co-maker or co-signer is necessary, complete the back of the application, providing all the requested information. Include at least(2) paycheck stubs for the oc-maker/signer. |

| 9. Complete the form regarding credit life or disability insurance, indicating which type of insurance you would like; if you do not elect either of the insurances, indicate this and sign the form. |

The credit committee, also appointed by the Board, is responsible for ensuring that loan requests and loan processing are effectively and fairly handled based upon policies and procedures established by the Board. |
